| * I find that athletes enjoy having an ice block after running around for an hour and a quarter. I have a selection of Zooper Dooper iceblocks to offer athletes after the training session. Do you give permission for me to give one to your child? If you have a peference please indicate when your child has registration confirmed. You may bring your own alternative for your child if you prefer. |

| Please collect your child punctually at 4.15PM as it is a condition of this Agreement that any parent who is late when collecting their child will be charged an additional twenty dollars for each and every 15 minute period that they are late. | All athletes are expected to follow a code of conduct that will ensure that the coaching sessions provide a safe, respectful and fun environment for all the athletes involved and the coach. Any athlete who engages in behaviour that is disrespectful towards others or jeopardises the safety of anyone involved will be deregistered. Placement in this course does not guarantee placement in future courses. 4. Once placement is offered payment must be received by the due date indicated in the placement email. If payment is not received it will be assumed you no longer want the placement and it will be offered to the next in the queue. 5. Any false or misleading information given in this form will result in the deregistration of the athlete. |
